New to Atrium Dental Studio? Please read the following information to get acquainted with our office:

Your First Visit:

Your new patient exam is scheduled separately from your cleaning and lasts about one hour. This includes:

• A complete digital scan of your mouth

• Updated X-rays

• One-on-one time with Dr. Salha to discuss your oral health, concerns, and smile goals

After the exam, we’ll schedule your cleaning based on Dr. Salha’s recommendations. Every treatment plan is personalized to you.

What We Need Before Scheduling?

We’re unable to reserve your appointment until we receive your new patient forms and information. Here’s how to get started:

1. Complete Your New Patient Forms
   Fill out your New Patient forms by creating a MyChart account utilizing this link —> Atrium Dental Studio Forms.
   You’ll create a MyChart account, and once your paperwork is submitted, we’ll set up your first visit.

2. Provide Recent X-Rays (if available)
   If you’ve visited another dental office recently, complete the attached release form and we’ll request your records. Please allow 7–10 days for processing.

3. Send Your Insurance Card
Email or text a clear photo of the front and back of your dental insurance card so we can verify your benefits before your appointment.
